How do I obtain a quote?
What if my dentist is not a contracted provider?
Many Dentist DIRECT plans offer in and out-of-network benefits. Remember, you can save money by receiving care from a contracted provider. Click here to see if your dentist is a contracted provider. You may refer your dentist to us through our website. If they meet our credentialing requirements and are willing to accept our contracted fees, they can become a contracted provider.

If we enroll in Dentist DIRECT, will my employees have waiting periods?
Any employees covered on your current plan the day prior to starting with Dentist DIRECT, will not have any waiting periods as long as the proper information was submitted with your enrollment. Typically a copy of your most recent prior carrier invoice will be sufficient. Otherwise, waiting periods will depend upon the plan you have chosen to purchase. There are plans available with no waiting periods for any services.
How do I obtain additional supplies for my employees?
Contact your designated account manager or simply call our general service line at 1-866-696-6527.
When will I receive my bill each month?
Invoices are generated between the 15th and 20th of every month. Payment is due on the first of the month.
When will our ID cards arrive? How do I order replacement cards?
Typically, ID cards arrive via mail within two weeks. Replacement cards can be ordered by calling 1-866-696-6527. Members can also print temporary ID cards by logging into the Member Login.
What if someone loses their card?
Members can login through the Member Login and print a temporary ID card. You can also request a new card vial email or by calling 866-696-6527.
When do new employees become eligible for the dental plan?
New employees are eligible to be added to the plan as soon as they fulfill the eligibility waiting period set forth by the employer. Employees may enroll in Dentist DIRECT within 30 days after they become eligible.
Is the employer required to contribute towards the premium?
No. Dentist DIRECT offers voluntary plans requiring no employer contribution. We also offer contributory plans offering premium discounts for employer contributions. See your rate proposal for details.
